For Practice Nurses and Staff
The Southern Nurse Network provides an avenue for Primary Health Care Nurses working in the southern and central areas of Adelaide to share ideas, network and expand their knowledge regarding education specific to their needs.
Topic: Supporting Elderly Access to Homecare & Advance Care Planning
This education session will focus on enabling primary care nurses to provide support to elderly patients at their practice and help them to access home care packages. The session will provide an outline on advance care planning and advance care directives. The presenters will discuss the components of good advance care planning and how to use conversation starters as a guide to formulating a comprehensive advance care plan.
Learning outcomes
Identify differences between aged care programs and understand how each program caters for different clients care needs.
Discuss pathways to accessing each program.
Explain the importance of advance care planning as a part of routine care and the person’s healthcare journey.
Discuss how to use an advance care planning approach in the workplace and understand practice nurses role in promoting it.
Outline advance care directives and discuss the legal framework.
